Is the green Delta Phase card always meant to be at the bottom when starting a new session? Ie shuffle red cards then add green to bottom, or after intro dream do you just shuffle it in with the red?

At the start of a session, it should be at the bottom of the deck. This is because Step 5 of Setup (pg. 5 of the rules) has you resolve Steps 4 and after on the Awakening card. One of those steps has you place the Delta Phase at the bottom.
